2025-2026 K-8 NEW STUDENT ENROLLMENT PACKET
The 2025-2026 enrollment packet may be completed with a PDF editor or printed and completed by hand. Please print single-sided!
An original birth certificate for the student, proof of residency within District 113A, parent/guardian identification, and a completed enrollment packet for each new student is required for enrollment.
Pre-printed packets will be available at each SD113A school building by January 27, 2025.
2024-2025 K-8 NEW STUDENT ENROLLMENT PACKET (DO NOT USE THIS PACKET FOR 2025-2026 ENROLLMENT)
Medical, dental and vision forms are not required at the time of your K-8 registration appointment but must be turned in directly to the school before October 15, 2024. Transfer students starting after the beginning of the school year have 30 days to submit all required health forms. Please contact the school nurse with questions.
